CHATTANOOGA — BlueCross BlueShield of Tennessee has recognized two Elizabethton physicians and a Johnson City provider group for outstanding care through its Medicare Advantage 2024 Quality+ Partnerships Program.

Dr. Jonathan S. Bremer and Dr. Abigail Gass, both of Elizabethton, earned a 4.5 out of 5 Star rating for delivering superior outcomes in preventive care and chronic disease management for Medicare Advantage members.

State of Franklin Healthcare Associates PLLC, based in Johnson City, was the only provider group in the region to achieve a perfect 5.0 Star rating.

The program highlights providers who improve in key quality metrics such as preventive treatment, chronic disease support, and medication compliance. Each honoree serves at least 25 BlueAdvantage PPO members.
